APPOINTMENTS
Patients are seen by appointment and on an emergency basis. Appointments
are available Monday through Friday, phones are on from 9:00-12:00
then on again from 1:00-4:30.. When making your appointment, please
describe your symptoms to our
scheduling staff. At the time of your appointment,
the doctor will try to not keep you waiting. Please provide us with
a telephone number where you can be reached during the day, in the
event that we need to change your appointment. If you are unable to
keep your appointment, please try to give us 24 hours notice. If you
would like your appointment rescheduled, we will offer you the next
available opening.
TELEPHONE CALLS
Our staff will take calls Monday through Friday, phones are on
from 9:00-12:00 then on again from 1:00-4:30. If you are calling
to speak to your doctor and he or she is unavailable, our staff
will deliver your message. Your doctor will return your call as
soon as possible. If you call outside of office hours, your call
will be taken by our answering service. When necessary, the on-call
doctor will be paged and will respond as soon as possible. Of course,
emergency calls will be handled promptly at all times of the day
or night.
PRESCRIPTION REFILLS
If you are calling to request a medication refill, please call during office hours approximately 3 days before the refill is needed. Please be ready to give your pharmacy's phone number to our staff. Refills on prescriptions will not normally be issued outside of regular office hours.
HOSPITAL AFFILIATIONS
Our physicians and psychologists maintain active staff privileges at Naples Community Hospital and North Collier Hospital.
RECORDS
Your medical records are confidential. If you wish to have copies of your office records released to someone outside of our practice, you must send us a written request authorizing us to do so. Please keep our office informed of any change in your name, address, employer, telephone numbers, and health insurance.
FEES AND INSURANCE
We believe that our professional fees are within the range of usual and customary charges for this region. Our doctors expect payment at the time of your office visit. All patients, including those covered under insurance plans with which we participate, are required to pay their co-payments and deductible at the time of service.
MEDICARE
We accept Medicare assignment on Medicare Medigap and Medicare-Medicaid insurance combinations. On all covered charges, Medicare will pay 80% of their fee schedule. We will take the appropriate Medicare adjustment and bill the remaining 20% of the charge to your secondary insurance. If you do not have secondary insurance, you will be responsible to pay the outstanding 20%. We will submit the charges to Medicare and your secondary carrier directly from our office.
PRIVATE INSURANCE AND MANAGED CARE PLANS
We participate in many insurance plans. We will bill your charges directly to these plans. We will send you a statement for any charges that the plan determines are the patient’s responsibility. If you are not sure if our doctors participate in your plan, one of our staff members will be happy to answer your questions. WORKERS COMPENSATION AND NO-FAULT
Our office submits claims directly to workers compensation and no-fault carriers. If you should have any questions regarding your claim, please contact our patient billing department.
